What We Do:
In many ways, the nurse is the center of the home health medical team. A nurse or, in some cases, a therapist, will be assigned as “case manager” for the patient’s care. He or she coordinates the plan of care your doctor has prescribed. This ensures that the patient gets the best care possible and meets their health goals.

A home health nurse sets up an individualized plan of care in consultation with the patient’s doctor. This plan is created with the patient’s goals in mind. Then, the home health nurse will educate patients and caretakers about illnesses and treatment. They monitor the patient’s health, symptoms and vital signs. They also provide wound care, ostomy care, intravenous therapy, and other medical services. Home health nurses assist with medication management and pain management. They are also able to detect and address early symptoms that could lead to hospitalization.

All of our home health nurses are licensed practical nurses or licensed vocational nurses. They hold their diploma or certificate from an accredited, state-approved program. They also hold licensure after passing the National Council Licensure Examination. They are CPR certified, a registered nurse with diploma from an approved nursing program or four-year Bachelor of Science in nursing, and hold licensure after passing the National Council Licensure Examination.
